Kids Aligners

Kids aligners are removable, transparent orthodontic appliances used to guide tooth movement in children, particularly during the mixed-dentition stage. They can be used for selected interceptive orthodontic problems, but they are not suitable for every child or every malocclusion. Current evidence shows that predictability varies with the type of movement and stage of dental development.

Contraindications/ difficult cases

Aligners are generally less appropriate as the sole treatment when there is:

Severe skeletal Class II or Class III discrepancy
Severe crowding requiring substantial space creation
Severe rotations
Multiple unerupted teeth requiring significant orthodontic guidance
Poor oral hygiene
Active untreated caries or periodontal disease
Poor patient/parent cooperation
Complex tooth movements requiring high biomechanical control

A growing child with a significant skeletal discrepancy may need growth modification rather than simply tooth alignment.

How aligners move teeth

Excellent compliance is essential; children must wear them as prescribed.
They may be less predictable for complex tooth movements.
Significant skeletal discrepancies may require functional appliances, expansion appliances or fixed orthodontics rather than aligners alone.
Expansion achieved with aligners may be less than the planned amount, particularly in posterior/gingival regions.
Retention is still required after active treatment.

Clear aligners can be a useful orthodontic option for selected children and adolescents, particularly for mild to moderate malocclusions, crowding, spacing, and certain tooth movements. Their advantages include better aesthetics, removability, easier oral hygiene, and greater comfort compared with conventional fixed appliances.
